Jayleigh Sage Maddox was born on Wednesday, Jan. 14, 2026, in Fort Smith, Ark., and died on the same day.

No formal service is planned at this time. Cremation is under the direction of Agent Mallory-Martin Funeral Home.

Survivors include her mother, Kaylin Caudle, of the home; father, Shane Maddox, of Van Buren, Ark.; three brothers, Greyson, Malechi and Declan; grandparents, Christy Burks and Jason Burks of Eufaula; great-grandparents, Olen and Rita England of Muldrow; and other family members.

She was preceded in death by her paternal grandfather, Earl Caudle II.
